The Tree Farm Inspection Record (004 Form)

The 2004 version of the 004 Form for Tree Farm inspections is now available. Although similar to the previous 004 Form in design, the new form includes a few notable changes:

  • Requirement for the field inspection date and the Tree Farmer interview date and type.
  • Requirement of an Inspector ID number.
  • Check boxes to indicate whether certification is recommended or approved.

The most significant change - the new 004 Form is now in five (5) parts. The fifth page of the 004 Form is the Auditor Verification Form, a checklist that helps determine whether a landowner is meeting the AFF Standards of Sustainability for Forest Certification. Check boxes are used to indicate the findings of each performance measure and indicator.

The refresher training course, which is a requirement for the 2004 Standard, reviews the new 004 Form and provides further instructions to correctly complete the form.

The Auditor Verification Form and the 004 Form have corresponding identification numbers and both forms should be kept together to help with the data entry of the information.
